Vibrant Dermatology sends appointment reminders as a courtesy, and it is ultimately the patient’s responsibility to know when their appointment date and time are.
At check-in, your credit card information will be obtained and kept securely until your insurance(s) have paid their portion and notifies us of the balance due, if any. After 30 days, if the bill remains unpaid, we will bill your credit card. Your ability to dispute a charge or question your insurance company’s determination of payment will remain unchanged. If you have any questions about our policy, please do not hesitate to ask.
Each time a patient misses an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, patients who do not show up for their appointment without a call to cancel at least 48 hours before the appointment time will be considered a NO-SHOW. This policy also applies to patients who call on the date of their appointment to cancel. Exceptions to this are at the discretion of the practice manager.
Vibrant Dermatology has the right to charge a fee of $75.00 for all missed medical appointments (“no shows”, day of cancellations). For missed cosmetic appointments, there is a $75.00 fee; however, if you have placed a deposit to hold the appointment and fail to attend, the deposit is forfeited.
“No Show” and late cancellation fees will be billed to the patient. This fee is not covered by insurance and must be paid in full prior to your next appointment.

PAYMENT AT TIME OF SERVICE: Payment is due in full at the time of service unless you are covered by Medicare or an insurance company with which we participate. You will be charged a $25 service fee for any returned checks, no exceptions.
INSURANCE: Patients will be asked to present their insurance card to the receptionist for copying upon check-in at the office each time they are seen for medical services. Please make it a point to bring your insurance card with you each time that you visit our office. Claims not paid within 30 days by your insurance company will become your responsibility. This includes claims that are denied for reasons such as, but not limited to, missing referrals, expired insurance, and incorrect primary or secondary insurance given. You will receive a statement for these services, and you will need to contact your insurance company for reimbursement.
For those patients covered by insurance plans with which we ARE participating providers, all co-payments, deductibles, and other non-covered services are due at the time of service. We will file your insurance claim with the insurance company. If your insurance coverage changes to a plan with which we ARE NOT participating providers, we will require payment in full at the time of service, and we will file your claim to the insurance company as a courtesy. Any charges that are not paid by your insurance company are your responsibility. Your insurance policy is a contract between YOU and your insurance company. Any pre-certifications of procedures or testing are your responsibility. Please let us know in advance if your insurance company requires this.
COLLECTIONS: Please note, if payment is not received from either you or your insurance company within 60 days from the date of service(s), your account will be considered delinquent and subject to referral to an outside collection agency.
Insurance Deductibles:
At check-in, your insurance policy information will be obtained and stored in our secure system. Vibrant Dermatology will run an eligibility report to ensure your insurance is active and to check if there are any deductibles or copays associated with the policyholder’s account.
If I have a deductible through my insurance policy, Vibrant Dermatology will charge the credit card on file for any balance that remains after my insurance company has completed the claim. I understand that all balances MUST be collected within 90 days from the appointment or be subject to collections.
